Personally, I'd choose "She has had no sleep in nearly 24 hours" but "She has trouble speaking with people in town" is also valid in this case. She could have trouble speaking with people in town maybe because she's not very outgoing, but the fact that she's had no sleep in nearly 24 hours means she has something on her mind that's bothering her.
