point slope form

y-y1=m(x-x1)

y-5 = 1/2 (x-1)

distribute

y-5 = 1/2 x -1/2

add 5

y = 1/2 x -1/2 +5

y= 1/2 x + 4 1/2

this is slope intercept form  slope=1/2  intercept = 4 1/2  or 9/2
