Tattoo at a place of residence, the facility _________
a) Must be a separate room (with a door so that when shut separates the room from the living area), have all necessary equipment required by the Agency, including chemicals and sharps containers
b) Must be clean, and floors in home must be non-porous, and licenses must be displayed
c) Must have identifying house number or sign indicating the location of the facility, have an entry that is separate from the living area of the home, have solid walls extending from floor to ceiling and must be equipped with the structures, accommodations, and equipment which the agency requires for all facilities.
d) Must display a demonstration permit, have a sign indicating location of the facility, meet all industry standards
