If any C5 substance is purchased by one individual more than 5 times within any 12 month period, the pharmacist will:
a) Issue a warning to the individual
b) Notify the individual's primary care physician
c) Refuse to dispense further until consulting with the prescriber
d) Report the activity to the appropriate regulatory authority
