A prescription may be transferred between pharmacies for the purpose of refill dispensing by telephone, or by facsimile or electronic means as provided in provided that:
a. The sending pharmacy invalidates the rx as of the date of transferred
b. on the back of invalidated prescription, the date of transfer, the name/address or store identifier of the pharmacy to which the prescriptions was transferred, the name of the pharmacist/intern/extern/ to whom prescription was transferred, the initials of the pharmacist/intern/extern issuing the prescription order
c. on the front of the invalidated prescription, the date of transfer, the name/address or store identifier of the pharmacy to which the prescriptions was transferred, the name of the pharmacist/intern/extern/ to whom prescription was transferred, the initials of the pharmacist/intern/extern issuing the prescription order
