If a mentally ill person requiring medical treatment is taken to UF Health Jacksonville by
JSO or JFRD personnel, the investigating officer will complete and submit a State of
Florida Form ______________________________________________ to hospital personnel.
After the medical treatment is rendered, hospital personnel will arrange for
transportation of the mentally ill person to the nearest mental health receiving facility.
Police personnel will not provide transportation.
